About YTP

The Youth Transition Program (YTP) is grant funded program which facilitates a partnership between the local Vocational Rehabilitation (VR) office and a school or district.

Districts that have YTP grants will have a Youth Transition Specialist on site who works directly with students who have disabilities that present a barrier to employment. This individual serves as a liaison between the school and Vocational Rehabilitation to facilitate student’s successful transition from school into employment or post secondary training programs.

Youth Transition Specialists work directly with district personnel to identify target students then provide a variety of services that support transition which will always include an application for VR services. Depending on the district and community, other services may include instruction on career development, career counseling, employability skills, or job application skills. Many of these services are identified though collaboration with the VR counselor and district staff such as a counselor of special education teacher and intended to mitigate barriers to employment.
